When to use the online form

Use the form for:

✅ Routine appointments
✅ Ongoing problems
✅ Medication queries
✅ Sick notes
✅ Test results or follow-up

👉 This is the main way to request an appointment

You do NOT need to phone us after submitting a request


What happens after I submit a request?

  • Your request will be looked at within 2 working days
  • We will usually contact you by email or text
  • You may receive:
    • Advice on what to do next
    • A booking link for an appointment
    • An appointment date if availability is limited

⚠️ Important:
If you are sent an appointment date, you must confirm within 2 days, or the appointment may be cancelled.


What if I can’t attend or have problems booking?

  • Reply to the text message you receive, or
  • Telephone the surgery and we will help

If you are struggling to use the form, our reception team will be happy to show you how.


What if I am unable or do not want to complete the form.

You can still:

  • Telephone the surgery.
  • Make a request in person at the reception desk.

When we receive your request (by telephone or in person), our reception team will assess your needs and may direct you to other services where appropriate, such as Pharmacy First.

If you require a non-urgent appointment at the practice, the receptionist can complete the request form on your behalf.

All requests, including those completed by reception, are reviewed by our team and responded to in the order received. We aim to contact you within two working days.


If no appointments are available

We do not have unlimited appointments, so we may sometimes advise other safe NHS options, such as:

  • Pharmacy First, where pharmacists can assess and treat common conditions
  • Urgent Treatment Centres, if your problem is urgent and we are at safe capacity
  • Being added to a routine waiting list if appropriate

Our reception staff are trained to follow guidance agreed with our clinicians. They may ask questions to make sure your request is handled safely and appropriately.
