What is SystmOnline
SystmOnline allows patients to view their electronic medical record, request repeat medication and book appointments available online.
Register for SystmOnline
If you haven’t registered for SystmOnline you will need to request a username and password from the practice. You can do this by printing and completing the relevant form below, please then bring the completed form to reception with up-to-date photo ID such as Driving Licence or passport. If you do not have a printer you can obtain a form at either of our practices. We will need a form for each patient including children.
Adult Application for Online Access. For patients aged 16+ You will need to bring the completed form and an up to date photo ID such as driving licence or passport
Child Application for Online Access For children aged under 11
Third Party Application for Online Access For a person needing access to another adults online services. This can be used for Carers and people with power of attorney etc.
Children aged 11 – 15 years old
PLEASE NOTE: Access to a child’s SystmOnline will automatically cease once the child reaches the age of 11 years. Medication requests will need to be made via email to overdale.reception@nhs.net It is generally considered that a child of 11 has the capacity to consent to who should have access to their medical record and can choose to withhold information from a third party. Giving online access to a parent/guardian could lead to disclosure of this information without consent from the young person. Wherever possible a clinician would always encourage a young person to involve whoever has parental responsibility in any treatment, but the clinician will decide as to whether a child is Gillick Competent and/or whether they meet Fraser Guidelines. These are terms within the NHS to determine if a young person it mature enough to make their own decisions. Once the child reaches the age of 16, online access can be given direct to the patient. The patient will need to complete an adult online application form and provide photo ID

Try the NHS App
If you’re a patient at our practice, you can use the NHS App to access a range of NHS services on your smartphone or tablet.
It doesn’t replace existing services. You can still contact us in the usual ways.
But, once you have verified your identity in the app, you will have easy, 24/7 access to a growing range of health services and information. You can:
- order repeat prescriptions
- access NHS 111 online
- find NHS services
- view your health records
- book appointments
- and much more…

If you have any problems using the NHS App, you can select ‘help’ in the top right-hand corner of the app or visit nhs.uk/helpmeapp.
PLEASE NOTE: if using the NHS App you WILL NOT be able to view your full medical record if you have not completed the high level security check within the App. See image below for details.

Forgotten your password?
If you have forgotten your password or have been locked out of your account you will need to attend the practice in person. Please remember to bring your photo ID.
