New Patients

Overdale Medical Practice welcomes new patients living within our practice area.

Before registering please ensure your postcode falls within our practice boundary.

PLEASE NOTE – the practice no longer accepts new patients living in Spondon. If you are living outside of the practice area you will not be eligible to register with us and you will need to register at an alternative surgery . For alternative GP practices please visit Find a GP – NHS

How to register

Once you have checked you live within our practice area please complete the online registration.

Online access to medical record

By registering you will have access to:

  1. Your prospective (future) medical records.
  2. Book some appointments.
  3. View past appointments and cancel any upcoming appointments.
  4. View test results
  5. Order repeat medications

Please print and complete the age appropriate form below and bring to surgery with your Photo ID.

Adult Application for Online Access For patients aged 16+ Photo ID MUST be provided.

Child Application for Online Access For children aged under 11 Photo ID of parent/guardian and child’s birth certificate or form of ID MUST be provided. Click this link for information on children aged 11-15

Carers Form

Please let us know if you are carer by completing this Carers Form. An informal carer is anyone, including children and adults who looks after a family member, partner, friend or neighbour who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid.

Temporary Residents

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. 

If you are residing within the practice area for up to 3 months and need to receive medical treatment, you can also register as a temporary resident. This can be helpful if you’re living away from home short term and do not want to change your home GP Surgery. You can be registered as a temporary resident for up to 3 months. After 3 months, you’ll have to reapply to register as a temporary resident or become a permanent patient.

To register you’ll need to fill out a temporary services form (GMS3), available from reception.
