Our records show you have already signed up for online access services via SystmOne, Airmid or NHS App. However, you do not currently have access to view your medical record from November 2022. By having access you can read your consultations with our clinicians, view blood results and view hospital letters plus much more

We are aware that not everyone will want access to their full record at this time, and some patients may have valid concerns about their personal medical records being made available on their smartphones or online, especially if other people have access to the same devices. Which is why we wish to obtain your permission first.

Please let us know your choice by completing the following forms:

• Please complete this Opt IN Form if you would like access to Full Prospective Online Medical record. As you already have online access you will not need to provide ID for this service.
• Please complete this Opt OUT Form if you DO NOT wish to have access to your Full Prospective Online Medical Record. If at any point you change your mind please contact the surgery.

PLEASE NOTE: if using the NHS App you WILL NOT be able to view your full medical record if you have not completed the high level security check within the App. See image below for details.
